George C. Grape Community Hospital is partnering with other community health providers over the next few months to update the Fremont County Community Health Needs Assessment.

The goal of this assessment update is to understand progress in addressing health needs cited in the 2019 and 2022 CHNA reports, as well as collect up-to-date community health perceptions and meet federal regulations.

Your feedback and suggestions regarding community health delivery are imperative to collect to be able to complete the 2025 CHNA and implementation plan updates.
